  3. Check the appropriate box to indicate method of payment desired. Where credit card is selected, please include the Visa or MasterCard number the work is to be charged to and the expiration date of the card. Where check or money order is selected, please include your check or money order with the unit when it is returned. Note make all checks payable to: Superscope Technologies, Inc.

  4. Include the completed form (and check or money order where applicable) with the unit and ship you unit to the address below:
